Official forms only
No homemade paperwork. Client data, medical answers, and signatures are overlaid onto the original association PDF form — the document layout stays untouched. Forms are available to each instructor according to their certification. PADI and NDL are supported today; forms for other associations are added at instructors’ request.
A medical statement recognized worldwide
The medical questionnaire is built on the form recognized by dive associations around the world (the RSTC/WRSTC standard; sources — DAN, UHMS, CMAS). Required for Intro Dive; for Fun Dive, it’s your decision.

On your device — the client fills out and signs right on your phone; no internet required. Or by QR — an individual code for each diver, so the whole group fills out forms at the same time on their own phones while you track each form’s progress on your screen in real time.
Your dive center’s own terms
Attach a page with your dive center’s own terms to any form — as a separate page after the official form, signed by the client. The official form itself is never modified.
Every signature verified
Every PDF includes a Signature Verification block: exact signing time, GPS coordinates, device. A document whose origin can be proven.
Your client base, always at hand
Every diver stays in your searchable client base with a complete history of signed waivers. A returning client means confirming existing data, not filling out a new form: personal details and certification carry over automatically.
Collaboration without risking your client base
Hand work to a colleague or a staff instructor — the client is registered directly to your dive center: all client data goes to your base, and the instructor who ran the signing keeps no client contacts. The document is signed on a form bearing your dive center’s name; both sides keep the document history.
Works with no signal

Data stays on your device
Signatures, medical answers, photos, and PDFs are stored encrypted on your device only. The cloud gets the minimum — your client list and visit history, so your base survives a phone change.
